Nursing attention to Mental Health, despite the advances provided by the Psychiatric Reform, is still permeated by reductionist approaches. The use of nursing theories, including the Tidal Model, can contribute to the foundation and qualification of mental health care. This theory emphasizes person-centered care that involves empowerment and autonomy in life management. It also seeks to promote the recovery of the person's voice for their development and growth, therefore, their Recovery. The model presents the use of holistic assessment and its “Holistic Assessment” instrument as a basis for identifying situations within the world of experiences and their relationships with mental health needs. Thus, despite being a reference used in several other countries and given the absence of instruments that contribute to the clinical practice of nurses in mental health care in Brazil in the most diverse contexts of action, this study aims to adapt the instrument cross-culturally “ Holistic Assessment”, from tidal model, for use in Brazil. This is a methodological study of cross-cultural adaptation, following the methodology proposed by Beaton, which consists of 6 stages, namely: Initial translation; Synthesis of translations; Retrotranslation or Back Translation; Review by a committee of experts; Pre-test and Submission of the pre-final version to a Scientific Committee specialized in cultural adaptation. Translations and back-translations were carried out independently by 4 bilingual translators. The translation synthesis was carried out by the author and the supervisor using standardization and adequacy criteria. The back-translation synthesis was carried out with the help of another translator, by the author and supervisor. Evidence of content validity was obtained through a committee made up of 11 specialist nurses, checking semantic, idiomatic, cultural and conceptual equivalences, which were analyzed using the content validity index. The semantic content analysis of the adapted instrument was carried out through pre-testing with 15 users of the Psychosocial Care Center and 15 nurses from CAPS and the Family Health Strategy, using the survey method and response coding through a scale likert. The translation and cultural adaptation stages allowed linguistic adjustments to be made for better understanding and practical applicability of the instrument items for use in Brazil. The content validity index of the Brazilian version was 0.98. In the pre-test, the average degree of global understanding for users was 4.7, while for nurses it was 4.8. Furthermore, 100% (n=15) of nurses stated that the instrument presents clear information, sees its practical applicability and would use it in their clinical practice. It is worth noting that the items that had changes suggested during the process aimed to make the Brazilian version as similar as possible to the original version, without affecting the understanding of the target audience, thus guaranteeing quality in the final result. Therefore, the transcultural adaptation of the instrument was carried out appropriately, thus enabling the availability of the holistic assessment instrument to be used in nurses' clinical practice in mental health care in Brazil. The study has the potential to contribute to the advancement of scientific knowledge in the area of health and nursing, qualifying nurses' assistance within mental health care in Brazil through practices based on technical/scientific knowledge and based on knowledge specific to the profession, such as the use of nursing theories, including the Tidal Model and this Holistic Assessment instrument.
